Our mission is to provide support and advocacy for individuals and families struggling with substance use disorder. 

Why?

We believe that connection is the opposite of addiction, and through a foundation of love—free of judgment —we aim to foster recovery and healing. Through our work, we seek to reduce the stigma surrounding substance use disorder, ensuring that those we serve feel the profound love of God and find hope in their journey toward recovery.

Jake Cole was fun-loving, loyal, and adventurous—someone who truly lived life with an open heart. Known for his unforgettable smile, Jake had a rare gift: he made people feel seen. Whether he had known you for years or had just met you, he created connection effortlessly and left a lasting impression.

Jake loved music festivals, traveling, and the ocean—places where he felt free and fully alive. He was passionate about his work in the cannabis industry and carried a natural drive and ambition that pushed him forward. Wherever he went, Jake built community. He made friends easily, listened deeply, and remained fiercely loyal and protective of the people he loved.

Jake’s struggle with substance use disorder began in his teenage years. Like many, he often tried to carry that burden on his own, reaching for support at times but continuing to fight a deeply personal battle. Through it all, he never lost his spirit. He believed in second chances, in comebacks, and in the possibility of something better ahead. His journey reflects a truth we hold close: substance use is often rooted in pain and trauma—not weakness.

As his family, Jake taught us what it means to love fully, take chances, and hold onto hope even in the hardest moments. He showed us the importance of staying connected to those who love you and have your best interests at heart—and to truly make a living out of living. Most importantly, his life reminds us that even in struggle, no one is ever separated from God’s love.

Breaking Chains: The Jake Cole Outreach Fund was created to honor Jake’s life and carry forward his legacy. Our mission is to help individuals struggling with substance use disorder find freedom from what binds them—while reminding them they are never alone and never beyond God’s reach.

We are committed to breaking stigma, ending judgment, and disrupting generational cycles through compassion, education, advocacy, and tangible support. Through this work, we provide financial assistance, increase access to life-saving resources like Narcan, and work to create a deeper understanding of substance use disorder as a disease.

Above all, we want every person struggling to know they are worthy—exactly as they are—and deeply loved by God. That no matter how heavy the chains feel, there is always hope, healing, and a way forward.

Jake’s life reminds us that no one is beyond redemption, and no story is finished.
